Note: Application and payment must be submitted together and no earlier than two weeks before the desired start date.

Termination of Service Application

Request discontinuation water and/or sewer service when moving or no longer needing service.

Account Name Change Form

Used to update the account holder’s name due to marriage, divorce, or legal name change.

Mailing Address Change Form

Used to update the mailing address associated with your utility account.

Letter of Authorization Form

Allows a property owner to give a tenant or authorized agent permission to establish or manage utility service on their behalf.

Billing & Adjustment Requests

Customer Adjustment Request Form

For requesting billing adjustments related to leaks or unexpected water usage.
Customers must provide invoices, receipts, or photos showing completed repairs.

New Construction & Development Services

Residential New Connection Application

Required for establishing a brand-new water or sewer connection to a residential property. Fee calculation form can be utilized to help determine associated utilities charges.
