The Florida Adult Services Program
A division of the Florida Department of Children and Families is
dedicated to protecting vulnerable adults from further abuse, neglect,
or exploitation and enabling disabled adults to remain in the community.

Care Line
A program established for the security and well being of those living alone
who would like daily contact with someone who cares. To participate in this

Safety Phones for Seniors
Enhances the safety of senior crime victims and seniors at-risk from becoming
victims of crime with the use and access of 911 through cell phones. To

Mid-Florida Community Services
Community Care for The Elderly
This state funded program provides a community-based service, which offers
an alternative to nursing home or institutional care. To qualify for the
various services, the client must be 60 years of age or over and be functionally
impaired. Fees are assessed on a sliding scale basis, dependent on individual
income and current expenses. In many cases, no fee is assessed. However
contributions are welcomed. Service provided includes Case Management, Emergency
Alert Response, Home Delivered Meals, Homemaker Service, Personal Care,

Safe Return Program
Alzheimer's Association Safe Return is a nationwide identification, support
and enrollment program that provides assistance when a person with Alzheimer's
or a related dementia wanders and becomes lost locally or far from home.

Skilled Nursing Facility
Skilled Nursing Facilities (SNFs) are freestanding nursing homes that provide
nursing care,
personal care, and custodial care to people who are ill or physically infirm.
They are not part of a
hospital. Some SNFs are part of a continuing care retirement community (CCRC)
and are
governed through special contracts. There are two main types of nursing
homes in Florida -
Skilled Nursing Facilities and Skilled Nursing Units.

Assisted Living Facilities
Assisted Living Facilities (ALFs) are licensed facilities that provide housing,
meals, personal care services, and supportive services to older persons
and disabled adults who are unable to live independently. ALFs are intended
to be an alternative to more restrictive, institutional settings for individuals
who need housing and supportive services, but do not need 24 hour nursing
supervision.

Adult Family Care Homes
An adult family-care home provides a full-time, family-type living arrangement
in a private home
for up to five aged or disabled people who are not related to the owner.
The owner lives in the
same house as the residents and provides housing, meals, and personal services;
however,
services vary. Adult family care homes are required to be licensed and inspected
by the state of
Florida.

The Enrichment Centers serve the community residents who are 55 years and
older, or individuals with special needs. The Center is a focal point
of health information and community services, as well as a means for an
individual to socialize and maintain a healthy, active lifestyle. Call 597-6331

Home Delivered Meals
Mid-Florida Community Services offer this federally funded service of the
Older American Act Title III Program for persons 60 years of age or over.
The most needy of our community will be given priority for this service.
There is no fee for the service, but all contributions are very much appreciated.

Seniors in the Park
An activity that allows seniors to listen to speakers on health related
topics that concern seniors. Seniors are also able to socialize with other
seniors in a beautiful park setting. Snacks, coffee & juice provided. Pre-Registration

Alzheimers Association
The State of Florida currently funds 13 Memory Disorder Clinics that provide
comprehensive assessments, diagnostic services, and treatment to individuals
who exhibit symptoms of Alzheimer's disease and related memory disorders.
The clinics develop training programs and materials and conduct training
for caregivers, respite service providers and health care professionals
in the care of persons with Alzheimer's disease or related memory disorders.
Memory Disorder Clinics are also responsible for conducting applied
research that is service-related and selected in conjunction with the Department
of Elder Affairs. Clinics are established at medical schools, teaching hospitals,
and public and private not-for-profit hospitals throughout the state in
accordance with F.S., Chapter

The Alzheimer's Association - Florida Gulf Coast Chapter is part of a national
voluntary health agency dedicated to providing education programs and support
services to people affected by Alzheimer's disease in the Florida Counties

Serving Health Insurance Needs of Elders (S.H.I.N.E.)
The S.H.I.N.E. Program provides elders and their caregivers with
information and unbiased health insurance counseling. Through a network
of trained volunteer counselors, assistance is provided regarding
Medicare, Prescription Assistance, Long-Term Care Insurance, Medicaid,

Florida's Long Term Care Ombudsman Program
A volunteer based organization seeking to improve the quality of life of
vulnerable elders who live in long-term care settings, including nursing
homes, assisted living facilities and adult facility care homes. For more

Comprehensive Assessment and Review for Long Term Care Services (CARES)
CARES is a federally mandated nursing home pre-admission assessment program.
Persons who are applying for Medicaid nursing home care are assessed by
either a CARES nurse or social worker, and medically reviewed by a physician

Medication Use Safety Training for
Seniors
A program launched by The National Council on Patient Information and
Education, a non-profit coalition of more than 100 organizations working
to improve communication on the appropriate use of medicines has a
national education awareness campaign to promote safe and appropriate
medicine use among America’s older adults. For more information about

Seniors EyeCare Program
Ensures that every senior has access to medical eye care and promotes
annual, dilated eye exams. The program raises awareness about
age-related eye disease, including cataracts, provides free eye care
educational material and facilitates access to eye care at no out-of
pocket cost. The program is designed for people who are US citizens or
legal residents, are age 65 and older, have not seen an ophthalmologist
in three (3) years or more, and do not belong to an HMO or the VA. Call
the toll-free helpline 800-222-3937 to request free eye care educational
materials and determine if they qualify for care from one of EyeCare
America’s 7,500+ volunteer ophthalmologists nationwide.
